One-On-One Personal Care

Family members can rely on Heritage to be there when their loved ones need help at home. Heritage provides comprehensive one-on-one personal care by caregivers which have been thoroughly screened and are experienced in providing services tailored to the client's needs.  Caregivers will provide needed services on a continuing basis or after an illness or post-surgery. We will help the client to live independently wherever they call home.

Non-Medical Services

  • Assistance with personal care such as bathing, grooming, feeding and oral hygiene
  • Assist with exercise and walking
  • Meal planning and preparation
  • Transportation to run errands and keep appointments (transportation is subject to payor source and policy limitations)
  • Grocery shopping and prescription pick-up
  • Laundry and garment mending
  • Light housekeeping and linen changing
  • Assisting and facilitating the patient's transition from a hospital or other health care facility into their own home and returning to goal level health
  • Assisting the family to better meet the needs of the patient through counseling, teaching, and coordination of community services
  • Working with the hospital and medical staff in the development of home care services that meet the needs of the patient
  • Maintaining patient function appropriate to individual capabilities, lifestyle, and stage of life
  • Administering quality health care services for patients in the home when caregivers are unable or unavailable to provide those services
  • Providing patient and family instruction in the fundamentals of health, hygiene and safety as well as the prevention, control and treatment of disease


To be eligible for services, clients must be enrolled in the Centennial Care/Medical program.
